IMMUNIZATION POLICY & GENERAL HEALTH APPRAISAL FORM

We follow the Colorado Department of Public Health and Environment requirements for immunizations. Please click here to learn more about the required vaccine schedule. Children must have up-to-date immunization records and health appraisal forms in order to attend school. Children must have a health appraisal form for each required checkup per American Academy of Pediatrics guidelines. Children are not allowed at school without both of these forms in their file If a child is on a delayed vaccination schedule, the schedule must be in accordance with the Advisory Committee on Immunization Practices recommended catch-up schedule, and we must have written documentation of the dates the vaccines will be administered. If a child has a medical exemption, we must receive a Certificate of Medical Exemption form completed by a doctor with their Professional License Number.  The Boulder JCC does not accept religious or personal exemptions.

FINANCIAL ASSISTANCE

The Boulder JCC has a need-based scholarship program, which you may apply for through the FACTS system. Please note: There is a $40 application fee. We do our best to meet the needs of all our families but are often limited both by the amount of money raised for scholarships and by the number of families seeking assistance.  First-round applications are generally due in early springtime. Applications received after the 2025 deadline will be considered on a rolling basis, but awards will be limited based on whether there are any remaining funds available in the scholarship program. All registration fees will be delayed until notification of your scholarship status has been received. At that time, if a family chooses to move forward with their registration, the registration fees will need to be paid to hold their spot. 

The Boulder JCC receives a number of grants and donations to provide the resources for our need-based scholarship fund. The JCC Scholarship Fund is open to all families regardless of race, ethnicity, religion, or background. We are grateful for the generosity of JEWISHcolorado, Rose Community Foundation, and Temple Buell Foundation for their donations to our scholarship fund. Grant funding provided by JEWISHcolorado is specifically for families that identify as Jewish and are in financial need. The application for both of these funds is through the same FACTS system. Therefore, when you are filling out your FACTS information, you will be asked whether your family identifies as Jewish. It is important to note that the JCC Scholarship Committee does not use this information when making scholarship determinations as we abide by our nondiscrimination policy
